  • Medical Records Release

    This form is needed when you would like to request a copy of your medical records for your personal use or for review by other physicians.
    • Only the patient may authorize release of medical records.  In the case of a minor, only the parent or legal guardian may authorize the release of medical records.  Proof of Legal Guardianship may be requested in order to comply with HIPAA and other privacy requirements.
    • Please be specific and complete all fields.
    • While you can complete the form online, the form must be printed, signed and dated, and returned to our office.
